What is inside
- Limits of the machinery worksheet — intended use, and the reasonably foreseeable misuse that weak assessments skip.
- Hazard register (XLSX) — hazard, hazardous situation, persons exposed, lifecycle phase, and the risk estimate before and after measures.
- Three-step measure record — the measure adopted, and where a design-stage measure was rejected, the reason it was not reasonably practicable. This column is the one that matters.
