What is inside

  • Limits of the machinery worksheet — intended use, and the reasonably foreseeable misuse that weak assessments skip.
  • Hazard register (XLSX) — hazard, hazardous situation, persons exposed, lifecycle phase, and the risk estimate before and after measures.
  • Three-step measure record — the measure adopted, and where a design-stage measure was rejected, the reason it was not reasonably practicable. This column is the one that matters.
  • Iteration log — because every protective measure can introduce a new hazard, and a single-pass assessment is an incomplete one.
  • Standards-to-hazards mapping — which type-B and type-C standards cover which hazards, so that "applied in full" is demonstrable.
  • Software and data corruption assessment — the new Regulation angle, as a set of questions with somewhere to put the answers.
  • Summary report (DOCX) — the narrative document that goes into the Annex IV file.

Who it is for

Machinery manufacturers and design engineers, and anyone whose existing risk assessment was done against the Directive and now needs reviewing against the Regulation.

What it does not do

This is a document template, not advice and not a certificate. It does not identify the hazards of your specific machine or decide what is reasonably practicable for you. Compliance remains the responsibility of the manufacturer. Full disclaimer →
